Description
Anterior to the distal femur and proximal tibia, the patella sits embedded within the quadriceps tendon and continues inferiorly as the patellar ligament to the tibial tuberosity. Medial and lateral to the patellar ligament, the fibrous expansions of the patellar retinacula blend with the fascia and the joint capsule, forming a broad anterior reinforcing layer. Along the sides of the joint line, the medial (tibial) and lateral (fibular) collateral ligaments track from the femoral epicondyles to the tibia and fibula, positioned superficial to the articular margins they stabilize. An anterior knee view clarifies why the extensor mechanism fails in predictable ways. Patellar tendon rupture typically occurs at or near its inferior pole attachment or within the tendon substance, producing patella alta and loss of active knee extension on exam. Surgical approaches such as a midline anterior incision or medial parapatellar arthrotomy also rely on these same fascial planes and retinacular fibers, where imprecise handling can contribute to postoperative maltracking and anterior knee pain. Orthopaedic and sports medicine courses can use this plate to teach landmark-based palpation of the patella, tibial tuberosity, and collateral ligaments, then connect those landmarks to common injury patterns and physical exam maneuvers.
