Description
Arising from the postolivary sulcus of the medulla oblongata, the glossopharyngeal nerve (cranial nerve IX) is traced on the basal surface as it courses anterolaterally toward the jugular foramen. From this inferior perspective, CN IX sits superior and slightly anterior to the vagus nerve (CN X) and the cranial rootlets of the accessory nerve (CN XI), with all three converging at the petro-occipital junction. The cerebellar hemispheres and cerebellar tonsils lie posterior to the medulla, while the optic chiasm and optic tracts occupy the more anterior midline on the ventral diencephalon. Small rootlets and short intracranial segments read clearly against the ventral brainstem. Attention to CN IX in this view pays off when teaching the jugular foramen region, because it is where confusion between glossopharyngeal and vagal symptom patterns often starts. Lesions at or near the jugular foramen (schwannoma, meningioma, glomus jugulare tumors, skull base fracture) can produce an IX deficit with dysphagia, impaired gag reflex afferent limb, and loss of taste from the posterior third of the tongue, and the close adjacency to CN X explains frequent combined palsies. The proximal segment also anchors discussion of the tympanic nerve (Jacobson nerve) and referred otalgia, and it provides the anatomic basis for glossopharyngeal neuralgia along the tonsillar fossa and base of tongue.
