Description
Oriented from an inferior perspective, the gallbladder is presented with its fundus, body, and neck in the foreground, the serosal surface overlaid by fine capillary beds that radiate across the fundal dome and converge toward the neck. Along the medial aspect, the cystic duct exits the neck to join the common hepatic duct, forming the common bile duct as it courses inferiorly toward the duodenum. Arterial inflow (classically via the cystic artery) tracks along the superomedial surface near the neck, while accompanying venous channels drain back toward the portal venous system, their caliber increasing proximally as they leave the gallbladder bed adjacent to the inferior surface of the liver. An inferior view is the one surgeons mentally rehearse. During laparoscopic cholecystectomy, the cystic artery and cystic duct must be isolated and clipped within Calot’s triangle, where anatomic variation is common and bleeding from small subserosal vessels on the fundus or from a short cystic artery can rapidly obscure the critical view of safety. This angle also helps teach why inflammation in acute cholecystitis thickens the gallbladder wall and increases vascular fragility, and why traction on the fundus changes the apparent relationship between the ductal structures and the hepatic surface. Ideal for gross anatomy and hepatobiliary modules, this illustration supports operative anatomy teaching, board style questions on biliary duct confluence, and figures in surgical texts describing ductal injury and hemorrhage risk during cholecystectomy. It also reads well in patient education materials that explain bile storage and bile duct flow without sacrificing anatomical nomenclature.
