Description
Oriented from the cranial base, this inferior (basal) view renders the middle meningeal vein running with the middle meningeal artery across the endocranial surface of the temporal and parietal regions, then tracing toward the foramen spinosum at the greater wing of the sphenoid. Medial to the temporal poles, the brainstem and adjacent basal cerebrum frame the field, while cranial nerves emerge from the midbrain, pons, and medulla and course anteriorly and laterally toward their skull base exits. Blue venous channels communicate inferiorly with the pterygoid venous plexus in the infratemporal fossa, a key extracranial connection that parallels the arterial route of the maxillary system. This angle matters because it clarifies the skull base pathway where meningeal vessels traverse dura and bone and where venous channels can act as low resistance conduits between intracranial and extracranial compartments. Venous bleeding from a middle meningeal vein can accompany fractures near the foramen spinosum and may contribute to epidural hematoma when dural stripping disrupts both vein and artery. That emissary style communication to the pterygoid venous plexus also explains a potential route for spread of infection from odontogenic or deep facial sources toward dural venous pathways.
