Description
Shown in an isolated adnexal context, the uterine (fallopian) tube is represented from its proximal isthmus near the uterine cornu to the distal infundibulum and fimbriae adjacent to the ovary. The ampullary segment appears fusiform and distended, with a thin tubal wall and a fluid-filled lumen that expands laterally and inferiorly within the mesosalpinx of the broad ligament. Distally, the fimbrial end is functionally sealed, creating a blind pouch consistent with hydrosalpinx and explaining the trapped serous fluid. Fluid under tension. Hydrosalpinx most often follows salpingitis from pelvic inflammatory disease, where scarring at the fimbrial end obstructs normal tubal drainage while the tubal mucosa continues to secrete fluid, progressively dilating the ampulla and flattening mucosal folds. That anatomy matters in infertility workups: a distended tube not only blocks gamete transport but can reflux inflammatory fluid into the uterine cavity, lowering implantation rates in IVF. The same configuration also raises concern for torsion of the adnexa and for chronic pelvic pain, and it guides operative planning when salpingectomy or proximal tubal occlusion is considered before assisted reproduction.
