Description
Anterior sectioning through the right ventricle exposes the trabeculae carneae as irregular muscular ridges along the free (anterior) wall and extending toward the interventricular septum medially. Between these ridges, the ventricular recesses and the coarse meshwork of trabeculation define the chamber’s inner contour. Superiorly, the trabeculated inflow portion transitions toward the smoother outflow tract (infundibulum) leading to the pulmonary valve, while papillary muscles project from the ventricular wall and give rise to chordae tendineae directed superiorly to the tricuspid valve leaflets. Orientation is anterior, but the relationships remain classic: papillary muscles sit deep to the valve annulus and tether the cusps during systole. Right ventricular trabeculation matters because it marks the functional divide between inflow and outflow and provides key landmarks used in echocardiography and operative planning. Hypertrophy of the trabeculae carneae and papillary muscles can contribute to right ventricular outflow tract narrowing in some forms of congenital heart disease, and the coarse internal architecture is central when distinguishing a morphologic right ventricle from a left ventricle in complex congenital anatomy. Arrhythmogenic right ventricular cardiomyopathy also draws attention to this region, where wall thinning and fibrofatty replacement may alter the appearance and mechanics of the trabeculated myocardium.
