Description
Colored anatomy isolates the lingular segment (segmenta bronchopulmonalia linguares) on the anterior aspect of the left lung, positioned on the anteroinferior portion of the superior lobe just inferior to the cardiac notch. Superiorly, the apex pulmonis rises above the clavicular level, while the lingula projects inferomedially toward the pericardium and left ventricular impression. Laterally, the lingular surface continues into the costal surface of the upper lobe, and inferiorly it approaches the oblique fissure separating the superior lobe from the inferior lobe. Segmental boundaries are implied by the highlighted color, framing the lingular division within the left upper lobe. Lingular anatomy matters because it behaves like the left-sided analog of the right middle lobe, with dependent anterior positioning that predisposes it to atelectasis and focal infection when ventilation is impaired. Radiologists and pulmonologists often localize lingular pneumonia on frontal chest radiographs by loss of the left heart border (a classic silhouette sign), and bronchoscopy targets the lingular bronchus when suctioning secretions or sampling for culture.
