Description
Cut through the skin in cross-section, the illustration centers on a dilated pilosebaceous unit whose infundibulum is distended by a dark keratin plug consistent with a blackhead (open comedone). The stratum corneum and viable epidermis form the superficial roof of the widened follicular ostium, while the follicle descends inferiorly into the dermis toward the bulb. A lobulated sebaceous gland empties into the follicular canal on its posterolateral aspect, and a coiled eccrine sweat gland sits deeper in the reticular dermis with its duct coursing superiorly toward the surface. Small-caliber arterioles and venules run within the dermis adjacent to fine cutaneous nerve fibers, above a hypodermis dominated by adipose lobules. Follicular obstruction is the first visible step in acne pathogenesis, and this close pore-level view clarifies where hyperkeratinization and retained sebum accumulate before inflammation develops. It also helps differentiate an open comedone from a closed comedone (whitehead), where the follicular orifice remains capped and the plug stays pale beneath the epidermis. Misplaced pressure during extraction compresses the perifollicular dermis and can rupture the follicular wall, seeding keratin and Cutibacterium acnes into the dermis and converting a simple comedone into an inflamed papule or nodule. Small lesion, big consequences.
