Description
Seen from the right lateral aspect, the rectum occupies the posterior part of the lesser pelvis, continuing inferiorly from the sigmoid colon at the rectosigmoid junction and terminating at the anorectal junction. Its sacral flexure follows the concavity of the sacrum and coccyx, while the perineal flexure angles anteriorly as it approaches the pelvic floor. Posteriorly the rectum lies against the presacral fascia, and anteriorly it relates to the prostate and seminal vesicles at mid and lower levels, with the rectovesical pouch and peritoneal reflection draping over the superior third. For teaching and clinical planning, a true lateral profile makes the rectal curves easy to read and aligns well with how surgeons and radiologists think about the pelvis in the sagittal plane. That matters during total mesorectal excision for rectal cancer, where staying within the mesorectal fascia and respecting the anterior plane near Denonvilliers’ fascia helps protect autonomic nerves contributing to urinary and sexual function. It also maps cleanly to digital rectal examination landmarks and to endoscopic advancement, where the sacral curve and anorectal angle predict resistance points and looping tendency.
