Anteriorly oriented, the cystic duct (ductus cysticus) is traced from the neck of the gallbladder to its junction with the common hepatic duct, forming the common bile duct inferior to the porta hepatis. Along the duct lumen, the spiral valves of Heister appear as mucosal folds that course obliquely, creating a corkscrew profile rather than a smooth channel. Medial to the gallbladder neck, the cystic duct angles posteromedially before meeting the common hepatic duct, a relationship that frames the usual layout of the extrahepatic biliary tree. Surgical anatomy hinges on this segment. During laparoscopic cholecystectomy, correct identification and clipping of the cystic duct in relation to the common hepatic duct and common bile duct helps prevent bile duct injury, still a feared complication when the biliary confluence is misread or a short cystic duct is assumed. The valves of Heister also explain practical difficulties in cannulation during intraoperative cholangiography and can complicate endoscopic passage of a guidewire when stones or inflammation narrow the lumen. Small structure, big consequences.
