Description
Running within the broad ligament, the uterine artery approaches the uterus at the level of the cervix and then ascends along the lateral uterine wall before giving circumferential arcuate arteries that course within the myometrium. From these arcuate vessels, radial arteries project medially toward the endometrium, where they continue as straight arteries to the basal layer and spiral arteries to the functional layer. Superiorly, anastomoses with the ovarian artery typically appear near the uterine fundus, while inferiorly the uterine artery relates closely to the ureter as it crosses anterior to it near the uterine cervix. Orientation is clear. Branching hierarchy is the point. This vascular layout matters when you need to understand uterine bleeding, placentation, and surgical hemostasis. During hysterectomy or uterine artery ligation, the artery’s course at the cervicouterine junction and its relationship to the ureter (water under the bridge) drives safe clamp placement and helps explain ureteric injury patterns. Within the endometrium, the distinction between straight and spiral arteries underpins menstrual shedding and helps frame disorders such as abnormal uterine bleeding and postpartum hemorrhage when spiral artery remodeling fails.
