Within the gestational week four conceptus, the embryo sits in a pronounced C shape with a disproportionately large cranial pole positioned superiorly and anteriorly to the developing trunk. Paired pharyngeal (branchial) arches form segmented ridges along the lateral lower head and future neck region, separated by pharyngeal grooves on the external surface. Caudally, the tail bud extends inferiorly beyond the primitive trunk, while the umbilical connection and early extraembryonic membranes lie ventral to the embryo within the amniotic sac. Early limb buds are minimal or absent at this stage. Small scale, big consequences. Week four is when clinicians and educators anchor the transition from trilaminar disc to a recognizable embryo, because the pharyngeal apparatus will pattern structures of the face, external ear, larynx, and great vessels. Disruption of neural crest migration or arch artery remodeling at this window underlies classic congenital anomalies such as first arch syndromes (micrognathia, mandibular hypoplasia), second arch-related defects in auricular development, and branchial cleft cysts or fistulae along the anterior border of the sternocleidomastoid.
