Description
Anteriorly, the epiglottis forms a leaf-shaped rim over the laryngeal inlet, sitting superior to the vestibular folds and true vocal folds within the laryngeal cavity. Medial margins of the vocal folds define the rima glottidis, while the laryngeal ventricle lies laterally between the vestibular (false) fold superiorly and the vocal fold inferiorly. Inferior continuation into the trachea is evident, with the airway lumen passing caudally beneath the glottic level in a sagittal or parasagittal section. This is a mucosa-forward view, with the cartilaginous framework implied by the contour of the supraglottic and subglottic airway. For airway and voice teaching, this sectional perspective matters because it aligns closely with what clinicians conceptualize when interpreting endoscopic findings and planning interventions around the glottis. Edema at the level of the vocal folds, a subglottic stenosis ring, or a supraglottic mass all narrow different segments of the same continuous lumen, and their functional consequences differ even when symptoms overlap. Intubation-related injury often localizes posteriorly at the vocal processes and interarytenoid region, but the functional bottleneck remains the rima glottidis, so understanding its boundaries and adjacent recesses helps explain stridor, dysphonia, and aspiration risk.
