Superiorly and slightly posteriorly lies the coccygeal direction of the intergluteal cleft, while anteriorly the perineum continues toward the scrotal root across the perineal body. Laterally, the perianal skin blends into the gluteal regions, framing the anal verge as the key surface landmark for deeper structures of the anal canal and external anal sphincter. Surface anatomy of the outer anus matters because pathology often announces itself at the verge before it is detectable proximally.
