Description

Anterior crural anatomy is presented with the tibia positioned medially and the fibula laterally, framing the anterior compartment of the male lower leg. The tibialis anterior lies immediately lateral to the tibial crest, while extensor hallucis longus occupies a deeper, more central plane between tibialis anterior and extensor digitorum longus, which spreads laterally toward the fibula. The anterior tibial artery and its venae comitantes course in the deep anterior compartment on the anterior surface of the interosseous membrane, traveling with the deep fibular (peroneal) nerve, which typically sits lateral to the artery distally as both approach the ankle. For teaching and clinical correlation, the anterior compartment is where ischemia and neuropathy declare themselves early in acute compartment syndrome, with pain on passive toe extension reflecting involvement of the long extensors and compromised anterior tibial artery flow. The same neurovascular bundle is at risk during operative fixation of tibial shaft fractures and during anterior approaches to the ankle, where retraction or inadvertent injury can produce deep fibular nerve sensory loss in the first dorsal web space and weakness of dorsiflexion. The course of the anterior tibial artery toward the dorsalis pedis also anchors palpation and Doppler assessment when posterior tibial pulses are difficult to obtain. Common use cases include gross anatomy lab guides for the crural region, surgical anatomy plates for orthopedic trauma and ankle arthroscopy references, and vascular or sports medicine teaching materials covering exertional anterior compartment syndrome and peroneal nerve entrapment.
