Description
Centered in an anterior view of the male abdomen, the large intestine is rendered in its principal segments, beginning with the cecum in the right lower quadrant and continuing superiorly as the ascending colon to the hepatic flexure. The transverse colon crosses the upper abdomen from right to left, then turns inferiorly at the splenic flexure into the descending colon along the left flank. Distally, the sigmoid colon loops medially within the left lower quadrant toward the midline. Segmental continuity is clear. Clinical teaching often hinges on these positional relationships because pain patterns, physical exam findings, and procedural planning map to them: right lower quadrant tenderness raises concern for appendicitis arising from the cecum, while left lower quadrant pain frequently tracks with sigmoid diverticulitis. Endoscopists and surgeons also orient by the hepatic and splenic flexures, common points of angulation during colonoscopy and key landmarks during mobilization for colectomy. Watershed zones sit at the splenic flexure (Griffiths point) and rectosigmoid junction (Sudeck point), areas prone to ischemic colitis during hypotension or after vascular ligation. Anatomy drives decision-making.
