Description

Centered in the anterior knee, the patella sits within the quadriceps femoris tendon and overlies the distal femur, with the medial and lateral femoral condyles flanking it on either side. Inferior to the patellar apex, the patellar ligament descends to the tibial tuberosity on the proximal tibia, crossing the anterior joint line of the tibiofemoral articulation. Medial and lateral patellar retinacula blend with the joint capsule, while the fibular head lies more laterally and slightly distal to the lateral tibial plateau. Surface anatomy matters here. Anterior knee anatomy is where patellofemoral mechanics become visible: maltracking of the patella relative to the femoral trochlea concentrates contact pressure and underpins common patellofemoral pain syndromes and chondromalacia patterns. The patellar ligament and tibial tuberosity form a frequent pain generator in adolescent traction apophysitis (Osgood-Schlatter disease) and in adult overuse tendinopathy, and the relationship between patella, tendon, and tibial tubercle also guides assessment of extensor mechanism rupture. Prepatellar and superficial infrapatellar bursae occupy the soft-tissue plane anterior to the patella and distal tendon, explaining the focal swelling seen with kneeling bursitis and offering a clear teaching point when differentiating bursitis from intra-articular effusion.
