At approximately 22 weeks of gestation the fetus lies in a flexed attitude within the translucent amniotic sac, surrounded by amniotic fluid and enclosed by the uterine wall, with vascular placental tissue visible along the upper margin of the cavity. The head is disproportionately large relative to the trunk, the cranial vault smooth and thinly covered, with closed eyelids, formed nose and lips, and a well-modelled auricle of the ear. One hand is drawn up to the mouth while the knees are flexed toward the abdomen, and the fetal feet with separated toes lie to the side. The umbilical cord, its coiled umbilical vein and arteries suggested by red and violet vessels beneath the amnion-covered surface, loops across the face and neck before passing to the abdominal insertion. This stage sits at the threshold of extrauterine viability, which makes accurate depiction of week-22 proportions and skin translucency important for counselling around extreme preterm birth and for periviability discussions in obstetrics.
