Description
Plantar skin of an adult male foot fills the frame, with erythematous, vesicular change and macerated scale concentrated along the sole and extending into the toe pads. Interdigital spaces show peeling and whitening consistent with moisture-related breakdown, while superficial fissuring tracks along flexion creases and the medial arch. Involved areas sit superficial to the plantar aponeurosis and metatarsal heads, but the distribution follows the stratum corneum rather than deeper soft tissue planes. Inflammation is obvious. Dermatophyte tinea pedis (athlete’s foot) often presents in predictable patterns, and this plantar, vesicular morphology aligns with an inflammatory dermatophytosis rather than the classic interdigital-only form. Clinically, the eroded, macerated toe webs matter because they act as a common portal of entry for secondary bacterial infection, including cellulitis and erysipelas of the foot and distal leg, and they also coexist with or precede onychomycosis of the toenails. Distinguishing this from palmoplantar psoriasis or dyshidrotic eczema hinges on border activity, annular spread, and response to topical corticosteroids, which can mask dermatophyte infection (tinea incognito) and worsen the burden. Podiatry and dermatology teaching sets use this type of close clinical view to anchor pattern recognition of dermatophytosis on the plantar surface, and to discuss specimen collection from the active scaling edge for KOH preparation, fungal culture, or PAS staining.
