Description
Viewed from below, the left hepatic lobe is organized into its lateral division (Couinaud segments II and III) along the left margin of the liver, separated from the medial division by the left hepatic vein in the left intersegmental fissure. The visceral surface faces inferiorly toward the stomach and lesser omentum, with the left triangular ligament attaching superiorly and laterally along the diaphragmatic surface. Medially, the fissure for the ligamentum venosum and the porta hepatis sit closer to segment IV, while the umbilical fissure carrying the ligamentum teres runs anterior and inferior toward the free edge. An inferior angle is the working orientation for much of hepatobiliary surgery because it matches the operative exposure after liver mobilization and retraction. Segment II sits posterolateral and cranial, segment III lies more anterolateral and caudal, and that distinction guides limited left lateral sectionectomy for colorectal metastases, hepatocellular carcinoma, or living-donor graft procurement. Short. Segmental boundaries also matter for controlling inflow via the left portal pedicles and for respecting venous drainage to the left hepatic vein, a common source of bleeding during parenchymal transection.
