Description

Jejunum loops occupy the central and left upper abdomen as the middle segment of the small intestine, continuous proximally with the duodenojejunal flexure at the ligament of Treitz and distally with the ileum at a gradually less distinct transition. From an external profile, the bowel is suspended from the posterior abdominal wall by the mesentery of the small intestine, so the jejunal coils hang anteriorly and inferiorly with a broad arc of mobility. Mesenteric attachment runs obliquely from the left upper quadrant toward the right lower quadrant, with jejunal branches of the superior mesenteric artery and vein fanning toward the antimesenteric border. Mobile gut. Jejunal anatomy matters because its motility and mesenteric freedom shape both symptom patterns and operative planning. Closed-loop small-bowel obstruction can trap jejunal loops under adhesive bands or internal hernias, producing rapid distension and ischemia when mesenteric vessels are compromised. In abdominal surgery, surgeons often identify jejunum just distal to the duodenojejunal junction to measure bowel length, construct a Roux-en-Y limb, or choose a segment for feeding jejunostomy, and the vascular arcades and relative paucity of mesenteric fat compared with distal ileum guide segment selection.
