Description
Medial aspect of the spleen is rendered with attention to the visceral (medial) surface, where the hilum interrupts the otherwise convex contour and the peritoneal reflections converge. Along this surface, the gastric impression lies anterosuperiorly, while the renal impression occupies a more posterior and inferior position adjacent to the left kidney. Inferior to the hilum, the colic impression and the splenic flexure relationship are suggested by a broader concavity, with pancreatic contact inferred near the hilum where the tail of the pancreas approaches in the splenorenal (lienorenal) ligament. Deep notches and impressions create the “splenic base” topography referenced in gross anatomy descriptions. Clear spatial orientation matters. For teaching and clinical correlation, the medial view is where the spleen stops being a smooth purple ovoid and becomes an organ with neighbors that leave fingerprints. Appreciating the renal and colic impressions helps explain why left upper quadrant trauma can bleed into the peritoneal cavity yet also irritate adjacent structures, and why mobilization during splenectomy often focuses on the hilar region where the splenic artery and vein divide into segmental branches. This angle also sets the stage for understanding iatrogenic pancreatic tail injury during hilar dissection, a known complication when the relationship to the splenic hilum is underestimated.
