Description
Superior to the hyoid bone, the suprahyoid compartment is defined by the mylohyoid forming a muscular floor, with geniohyoid positioned immediately superior to it and the paired anterior bellies of the digastric running anteroinferiorly toward the intermediate tendons near the hyoid. Medially, the genioglossus and hyoglossus anchor the tongue to the mandible and hyoid, while the stylohyoid and posterior belly of the digastric course posteroinferiorly from the styloid region toward the lesser horn region of the hyoid. Laterally, the mandible frames the submandibular triangle, where the submandibular gland occupies the space superficial to mylohyoid, and the sublingual gland rests more anteriorly on the superior surface of mylohyoid in the floor of mouth. Nerve and duct relationships are implied: the hypoglossal nerve tracks forward deep to the posterior belly of digastric, and the submandibular duct runs anteriorly toward the sublingual caruncle. Clinical work in the neck often turns on these planes. Infection spreading from odontogenic sources into the submandibular and sublingual spaces (Ludwig angina) respects the mylohyoid “sling,” so whether a collection lies above or below mylohyoid changes the pattern of swelling and the urgency of airway management. The same anatomy guides submandibular gland excision and floor-of-mouth approaches, where protecting the marginal mandibular branch of the facial nerve superficially and the lingual nerve crossing the submandibular duct deep in the triangle is non-negotiable. Small distances matter.
