Description
Cutaneous layers are rendered in cross-section, from the stratified squamous epidermis (including stratum basale and the more superficial keratinizing strata) down into the papillary and reticular dermis and the subcutaneous adipose tissue of the hypodermis. An atypical melanocytic proliferation forms an irregular, pigmented tumor mass centered at the dermoepidermal junction, then extends inferiorly as invasive nests and cords into the dermis. Adnexal structures such as hair follicles with associated sebaceous glands and coiled eccrine sweat glands sit within the dermis, while small arteries and veins course deeper and flank the lesion. Depth matters here. Once melanoma breaches the basement membrane and enters the dermis, Breslow thickness and ulceration become the main determinants of staging and sentinel lymph node biopsy recommendations, because access to dermal lymphatics and microvasculature increases metastatic risk. The illustration also supports teaching of radial (intraepidermal) growth versus vertical (dermal-invasive) growth, and why invasion around follicular units can complicate margin assessment. A dangerous transition.
