Description
Layered around a cross-sectioned skeletal muscle fascicle, the perimysium forms a distinct outer sheath while the endomysium partitions and supports individual skeletal muscle fibers within. Each muscle fiber presents a sarcolemma boundary enclosing sarcoplasm packed with longitudinal myofibrils, the internal cylinders that generate the characteristic striated appearance at higher magnification. Neurovascular elements course through the connective tissue septa, with small arteries and veins running alongside nerve fibers that approach and branch toward the fibers. At one margin, the muscle’s connective tissue continuity tracks toward a bony attachment, reinforcing the proximal-to-distal linkage between contractile tissue and the skeleton. Clinically, this hierarchy matters because many “muscle” problems localize to a specific level: endomysial inflammation and fiber necrosis in polymyositis or dermatomyositis, perimysial scarring after strain, or microvascular compromise affecting perfusion during compartment syndrome. That connective tissue scaffolding is also the pathway for intramuscular injections and the corridor for spread of infection or hematoma. A short landmark. The depiction of sarcolemma, myofibrils, and sarcoplasmic reticulum supports teaching excitation-contraction coupling, where the action potential travels along the sarcolemma and into the fiber to trigger calcium release for actin-myosin interaction.
