Description

Anterior orientation frames the tunica mucosa of the stomach as the luminal lining, with rugae (gastric folds) running predominantly longitudinally and separated by shallow grooves. Surface simple columnar mucous epithelium continues into gastric pits (foveolae gastricae) that open into tightly packed gastric glands within the lamina propria. Deep to the mucosa, the muscularis mucosae forms a thin, wavy boundary before the submucosa, setting the plane along which the folds elevate. Attention stays on the mucosal microanatomy because this is where most clinically relevant gastric pathology begins and where endoscopists aim their biopsies. Gastritis and peptic ulcer disease typically track the pit and gland architecture, and Helicobacter pylori related injury often concentrates in the antrum, with inflammatory infiltrates and surface epithelial damage that alter the normal foveolar pattern. Small details matter. Early gastric adenocarcinoma and intestinal metaplasia are evaluated by how the mucosa reorganizes, not by changes in the muscularis externa.
