Oriented to the plantar aspect of the male hindfoot, the medial talocalcaneal ligament is shown spanning the subtalar joint from the medial tubercle and adjacent plantar surface of the talus to the sustentaculum tali and medial calcaneus, lying deep to the medial longitudinal arch. Immediately superficial and slightly plantar to this region course the deep flexor tendons in the tarsal tunnel, with flexor hallucis longus running in a groove inferior to the sustentaculum tali and flexor digitorum longus passing more medially before dividing toward the lateral four toes. The calcaneus sits posterior and inferior, the talus superior, and the midfoot extends anteriorly into the tarsus and metatarsus with the plantar ligaments bridging the intertarsal joints. Medial subtalar restraints matter when you are trying to explain or repair hindfoot valgus and subtalar instability after inversion or eversion trauma. This plantar-medial view clarifies how the medial talocalcaneal ligament relates to the sustentaculum tali and the flexor hallucis longus tendon, a relationship that becomes clinically concrete in tarsal tunnel decompression and in operative exposure around the subtalar joint where tendon irritation or entrapment can complicate recovery.
