Posterior anatomy centers on the pancreatic tail (cauda pancreatis) as it tapers laterally toward the splenic hilum, lying in the left upper quadrant and tracking along the posterior abdominal wall. From this dorsal perspective, the tail sits left of the pancreatic body and approaches the spleen, a relationship that also brings it into close proximity with the splenic vessels coursing along the superior border of the gland. Depth is the story here, with the cauda pancreatis typically positioned anterior to the left kidney and adrenal region and tucked near the splenorenal (lienorenal) ligament.
