Posteriorly oriented, the descending (second) part of the male duodenum is presented as a vertical segment to the right of the vertebral column, with its superior end continuous with the duodenal bulb and its inferior end curving into the inferior duodenal flexure. The pancreatic head lies medial to it, and the descending duodenum forms a shallow groove against this pancreatic tissue while remaining retroperitoneal along most of its course. Superiorly, the hepatoduodenal region sits near the first part of the duodenum, while the hepatic flexure of the colon lies more lateral and slightly inferior, draping in proximity to the right upper quadrant. Orientation is clear. This posterior perspective matters because the descending duodenum is the segment that receives the hepatopancreatic ampulla (ampulla of Vater) on the major duodenal papilla, a focal point for biliary obstruction, post-ERCP pancreatitis, and periampullary tumors. Surgical and endoscopic planning often hinges on understanding its fixed retroperitoneal position, the intimate relationship to the pancreatic head, and the way inflammation or a pancreatic carcinoma can tether or narrow the duodenal lumen. In trauma and acute hemorrhage, the second part is also a site where retroperitoneal duodenal injury can be missed on initial exam and later declared by bile-stained drainage or pneumoretroperitoneum.
