Description
Posterior anatomy of the left abdomen is rendered to orient the adult male descending colon (colon descendens) against the retroperitoneal body wall. The descending segment tracks inferiorly from the left colic flexure toward the sigmoid colon, lying lateral to the left psoas major and quadratus lumborum and anterior to the lumbar fascia. Peritoneal reflections at the lateral paracolic gutter frame its usual secondary retroperitoneal fixation, with the mesocolic attachment line suggested where the colon is tethered to posterior connective tissue. Orientation is the point. That relationship matters when mobilizing the left colon. During a left hemicolectomy or sigmoid resection, surgeons divide the lateral peritoneal attachments along the white line of Toldt to enter the correct avascular plane, separating colon and mesocolon from the underlying lumbar fascia and retroperitoneal musculature. Staying on the proper plane helps avoid traction or direct injury to retroperitoneal structures, including the left ureter and gonadal vessels, and limits bleeding from the posterior abdominal wall. Faculty will find this posterior spatial view useful for teaching peritoneal compartments and colon fixation in gross anatomy, colorectal surgery clerkships, and operative anatomy modules that cover medial-to-lateral versus lateral-to-medial mobilization.
