Description
Oriented from the right side, the cardiac base is dominated by the right atrium, with the smooth sinus venarum positioned posteriorly and the right auricle projecting anterosuperiorly along the atrioventricular groove. Superior and inferior to the atrial wall lie the caval entry regions for the superior vena cava and inferior vena cava, their attachment sites implied even though the surrounding vessels are absent. Inferior to the atrium, the right ventricle forms the anteroinferior contour, while the interatrial sulcus marks the transition toward the more posterior left atrial component of the base. Clean margins. No mediastinal clutter. This angle matters when teaching how the base of the heart is not the diaphragmatic surface but the posterior atrial surface, a distinction that underpins orientation on cross sectional imaging and in the operative field. Cardiac electrophysiology also benefits from this view: the sinoatrial node sits at the superior aspect of the right atrium near the SVC junction, and the crista terminalis separates the smooth sinus venarum from the pectinate muscle region, a common mapping landmark during atrial tachyarrhythmia ablation. Understanding these surface relationships helps predict where catheter or suture lines approach the caval sleeves and where injury can provoke sinus node dysfunction.
