Description
Centered on the anterior trunk of an adult male, faint salmon-colored macules and small maculopapules are scattered over the upper chest and shoulders, with a lighter extension onto the proximal upper arms. Lesions sit superficial to the dermis and are shown against otherwise intact skin, making their low-contrast borders and subtle elevation easier to appreciate. The distribution favors the thoracoabdominal region rather than the face or distal extremities, matching the classic “rose spots” pattern described in typhoidal Salmonella infection. Scale matters. Rose spots matter because they are one of the few early external clues to enteric fever when the dominant complaints are systemic, including stepwise fever, malaise, and abdominal discomfort, and the patient may not yet have prominent diarrhea. Clinically, these lesions are typically 2 to 4 mm, appear in crops during the second week of illness, and may blanch with pressure early, then fade within days, so timing and bedside technique influence what you document.
