Description
Oriented in anatomical position, a superior view of the right lung emphasizes the horizontal fissure (fissura horizontalis, minor fissure) as it courses transversely from the anterior margin toward the hilum, separating the superior lobe above from the middle lobe below. From this vantage, the fissure lies anterior and superior to the oblique fissure, which continues posteroinferiorly to partition the inferior lobe. The sharp inferior border is still appreciable as a thin rim outlining the diaphragmatic surface, sweeping inferiorly and laterally toward the costophrenic recess. Recognition of the right horizontal fissure matters because its presence and completeness vary, and that variability changes how air, fluid, and infection respect lobar boundaries. Middle lobe pneumonia often silhouettes the right heart border on frontal radiographs, while disease confined to the superior lobe does not, and the horizontal fissure explains that separation in a way students often miss when they only memorize lobes. For thoracic surgery and interventional pulmonology, the fissure also predicts the plane of dissection during lobectomy and influences postoperative air leak risk when the fissure is incomplete.
