Description
Centered at the gastroesophageal junction, the distal esophagus descends through the esophageal hiatus of the diaphragm to meet the gastric cardia, where the lower esophageal sphincter (LES) region is shown in a closed, tonically contracted state. Superior to the stomach, the tubular gullet courses inferiorly from the thoracic esophagus toward the abdomen, while the proximal duodenum continues distally from the pyloric canal. Medially, the lesser curvature leads up toward the cardia, and laterally the fundus domes beneath the left hemidiaphragm, defining the acute angle of His at the junction between esophagus and stomach. The right and left crura of the diaphragm encircle the esophagus at the hiatus, reinforcing the sphincteric zone. A closed LES is the main anatomic concept behind reflux prevention, and this view makes it easier to teach how diaphragmatic pinch and intrinsic smooth muscle tone cooperate to block retrograde flow of gastric contents into the distal esophagus. Small failures at this junction have big consequences. Reduced LES pressure contributes to gastroesophageal reflux disease and reflux esophagitis, while anatomic disruption from a sliding hiatal hernia displaces the junction superiorly and blunts the angle of His, weakening the anti-reflux barrier; in contrast, impaired relaxation in achalasia produces functional obstruction with proximal dilation.
