Description
Anteriorly oriented, the distal femur overlies the proximal tibia at the tibiofemoral articulation, with the patella and anterior joint capsule implied as the superficial landmark for the vascular plexus. Genicular branches course transversely and obliquely around the joint line, forming an anastomotic ring that concentrates medial and lateral to the patellar margins. Superior genicular vessels run inferiorly from the femoral side, while inferior genicular vessels ascend from the proximal tibia, converging toward the peripatellar arterial network. Small arteriolar branches arc across the anterior knee. Understanding the anterior genicular anastomoses matters when you are trying to preserve perfusion during approaches that work close to the patella and tibial tuberosity, including arthrotomy, tibial tubercle osteotomy, and exposure for total knee arthroplasty. Disruption of the peripatellar circulation can contribute to delayed wound healing and, in select cases, osteonecrosis of the patella after TKA, especially when lateral release or aggressive retinacular dissection compromises the lateral superior genicular contribution. This view also supports teaching the concept of collateral circulation around the knee when the popliteal artery is narrowed or occluded, where genicular arteries can maintain distal flow via periarticular channels.
