Description
Running superolaterally from the iliac crest and thoracolumbar fascia toward the inferior borders of the lower costal cartilages, the internal oblique (musculus obliquus internus abdominis) forms the intermediate layer of the anterolateral abdominal wall. From an anterior trunk view, its fiber direction contrasts with the external oblique, and its superior slips interdigitate with the lower intercostal musculature along ribs 10 to 12. Medially, the muscle transitions into an aponeurosis that contributes to the rectus sheath adjacent to rectus abdominis and the linea alba. Upper-limb and thoracic wall muscles may remain in the field, but the abdominal wall dominates. Clinically, the internal oblique matters because its aponeurosis helps form the inguinal canal roof and contributes to the conjoint tendon with transversus abdominis near the pubic crest, a key relationship when teaching direct versus indirect inguinal hernias. Fiber orientation also explains pain patterns and weakness after lower abdominal incisions and trocar placement in laparoscopy, where splitting along muscle direction can limit denervation and postoperative bulging. Palpation and resisted trunk rotation testing often target this layer, but the anatomy is easiest to understand when you can map attachments to the costal cartilages and the iliac crest.
