Description
Arising from the intracranial segment of the vertebral artery near the foramen magnum, the posterior meningeal branch courses posteriorly to reach the dura mater lining the posterior cranial fossa. A lateral brain view places the cerebellum and dorsal brainstem inferior and posterior to the cerebral hemispheres, with arterial channels in red tracking along the ventrolateral medulla and upper cervical cord before giving off the meningeal twig. Superficial venous pathways in blue drape the cerebral convexity and posterior fossa region, while yellow cranial nerves radiate from the brainstem, their root entry and exit zones clustered anterolateral to the pons and medulla. Small vessel. Big consequence. Dural supply in the posterior fossa matters because meningeal branches of the vertebral artery can participate in vascular lesions that present with headache, posterior fossa symptoms, or cranial neuropathies, and they form potential anastomoses with occipital and ascending pharyngeal contributions to the dura. This relationship becomes concrete in endovascular planning for posterior fossa dural arteriovenous fistula, where a vertebral meningeal feeder may sit uncomfortably close to medullary perforators, the posterior inferior cerebellar artery origin, and lower cranial nerve rootlets at the jugular foramen and hypoglossal canal.
