Description
Beneath the occipital scalp and nuchal skin, the rectus capitis posterior minor lies as a paired, deep suboccipital muscle running from the posterior tubercle of the atlas (C1) superiorly to the medial part of the inferior nuchal line on the occipital bone. Superficial to it, the trapezius and splenius capitis form the more posterior muscular envelope, while semispinalis capitis occupies the intermediate layer and must be traversed to reach the suboccipital compartment. Lateral to the rectus capitis posterior minor, the rectus capitis posterior major and the obliquus capitis muscles outline the suboccipital region around the C1 to C2 segment and adjacent posterior elements of the cervical vertebrae. Small, but specific. This posterior oblique, slightly lateral viewpoint matters because it matches the surgical and interventional corridor used when clinicians work near the craniocervical junction, where atlas anatomy, the occipital attachments, and the suboccipital muscle plane determine safe needle depth and trajectory. Rectus capitis posterior minor is also clinically tied to cervicogenic headache concepts through its fascial continuity with the posterior atlanto-occipital membrane and dura (the myodural bridge), a relationship discussed in manual therapy and in pain-focused anatomy teaching. Nearby landmarks such as the course of the greater occipital nerve over the inferior oblique and the vertebral artery in the suboccipital triangle are often taught in the same breath, and this layering helps keep those relationships straight.
