Description
Circumferentially arranged around the bronchial lumen, the mucosa forms the innermost boundary and transitions outward into a gland-bearing submucosa, followed by a discontinuous ring of hyaline cartilage plates that sit anterolaterally and posterolaterally within the bronchial wall. Bundles of smooth muscle (muscularis) course concentrically between mucosa and cartilage, tightening the airway when contracted. Small arteries and veins track through the submucosa and peribronchial connective tissue, while accompanying nerve fibers lie in close apposition to the muscular layer and vascular channels. Orientation is clear. This is a transverse cross-section. Airway wall layering is where many teaching errors begin, and this cross-section draws a clean line between structures that look similar on histology and imaging but behave differently in disease. Smooth muscle hypertrophy and submucosal gland hyperplasia are hallmark contributors to airflow limitation in asthma and chronic bronchitis, while the presence of cartilage plates helps distinguish bronchus from bronchiole when you are localizing pathology such as bronchiectasis or an aspirated foreign body on bronchoscopy. Peribronchial neurovascular bundles also matter during endobronchial biopsy, where depth and direction determine whether you sample mucosa or encounter bleeding from bronchial arteries.
