Description
Viewed from inside the opened cranial vault of an adult male skull, the superior cerebellar artery is traced as it arises near the terminal basilar artery and courses posterolaterally around the midbrain toward the superior surface of the cerebellar hemispheres. Along its proximal segment it runs inferior to the oculomotor nerve (CN III) and close to the posterior cerebral artery, then fans into cortical branches near the tentorial surface. Surrounding osseous landmarks include the cranial base, petrous temporal region, and margins of the posterior cranial fossa, with the maxilla, mandible, and dentition retained inferiorly for orientation. Orientation is unmistakably intracranial. This relationship is the point. At the basilar apex, the superior cerebellar artery sits at the center of aneurysm anatomy and endovascular navigation, and its proximity to CN III helps explain oculomotor palsy patterns seen with basilar tip and posterior circulation aneurysms. Its cerebellar territory also frames stroke localization: superior cerebellar artery infarcts can produce ipsilateral limb ataxia and dysmetria, and swelling in the posterior fossa carries a different risk profile than supratentorial edema. For microsurgical approaches, the artery’s course beneath the free edge of the tentorium guides corridor planning around the ambient cistern and cerebellomesencephalic fissure. Neuroanatomy and neurosurgery educators can drop this view directly into teaching on posterior circulation, basilar apex anatomy, and tentorial relationships, while authors of atlases or case reports can pair it with CTA/MRA or DSA images to clarify vessel identification in cross section.
