Description
Atlas (C1) anatomy centers on the sulcus arteriae vertebralis, a shallow groove on the superior surface of the posterior arch that cradles the vertebral artery as it courses medially and posteriorly toward the foramen magnum. From a superior perspective, the groove sits posterior to the superior articular facet and lateral mass, and it approaches the posterior tubercle near the midline while remaining continuous laterally with the region of the transverse process and foramen transversarium. The posterior arch forms the bony floor beneath the artery, with the spinal canal immediately inferior and the occipitoatlantal joint surfaces positioned anterior and lateral. Small variations in the bony rim over the sulcus can suggest a partial arcuate foramen (ponticulus posticus). Clinically, this topography matters in C1 posterior arch fractures and in instrumentation around the lateral mass and posterior arch, where drill trajectory and screw placement can endanger the vertebral artery if the sulcus is shallow or bridged by an arcuate foramen. A risky neighborhood. The relationship between the groove, lateral mass, and posterior tubercle also underpins discussions of vertebrobasilar insufficiency associated with rotational compression at the craniovertebral junction (often framed in the context of Bow Hunter syndrome), and it helps clarify why posterior approaches to C1-C2 stabilization demand meticulous attention to millimeter-level bony contours. Directional lighting on the superior surface makes the depression of the sulcus and the curvature of the posterior arch easy to interpret against adjacent articular surfaces.
