Description
Thalamus occupies the central diencephalon as paired ovoid masses of grey matter positioned superior to the midbrain and rostral to the pons. A lateral-oriented composition typically exposes the convex lateral thalamic surface abutting the posterior limb of the internal capsule, while the medial surface forms the lateral wall of the third ventricle and approaches the interthalamic adhesion when present. Posteriorly, the pulvinar overhangs the superior colliculus region, and the geniculate bodies sit at the posteroinferior margin where the thalamus transitions toward the metathalamus and optic tract. Thalamic topography matters because small vascular lesions can create outsized, localizing deficits. Paramedian infarcts from thalamoperforating arteries may combine altered arousal with memory disturbance, while posterolateral strokes in the territory of thalamogeniculate branches often produce hemisensory loss, ataxic hemiparesis, or the later development of Dejerine-Roussy (thalamic pain) syndrome. The fixed lateral viewpoint helps you relate the thalamus to the internal capsule and adjacent basal ganglia, a relationship that explains why nearby hemorrhage or edema can quickly involve corticospinal and thalamocortical fibers even when the thalamic nuclei themselves are only partly affected.
