Description
Gyrus rectus (straight gyrus) occupies the most medial strip of the orbital surface of the frontal lobe, running anteroposteriorly immediately medial to the olfactory sulcus. Lateral to it, the olfactory sulcus forms a shallow groove that typically lodges the olfactory bulb and tract, while medially the gyrus rectus abuts the interhemispheric fissure and the falx cerebri. Anteriorly it approaches the frontal pole, and posteriorly it blends toward the region of the anterior perforated substance at the basal forebrain. Orientation on the inferior frontal surface is unambiguous. For teaching orbital frontal anatomy, gyrus rectus is a reliable landmark for locating the olfactory groove and the basal frontal midline, where pathology often presents with subtle, nonspecific symptoms. Olfactory groove meningiomas commonly arise along the cribriform plate region and can compress the olfactory apparatus within the sulcus, producing progressive hyposmia or anosmia and personality or executive changes from orbitofrontal involvement; gyrus rectus provides the cortical reference point for that corridor. The fixed perspective along the medial orbital surface also supports surgical planning concepts, including subfrontal and endoscopic endonasal approaches where the relationship between the olfactory sulcus, frontal base, and midline structures must be kept straight to avoid unnecessary frontal lobe retraction and to anticipate CSF leak risk. Neuroanatomy instructors can place this illustration into modules on frontal lobe gyri and sulci, olfactory pathways, and basal forebrain topography, while authors of neurosurgical and neuroradiology texts can use it to label the gyrus rectus when discussing anterior cranial fossa tumors or anterior communicating artery aneurysm workup.
