Description
Superior medullary velum spans the interval between the right and left superior cerebellar peduncles, forming a thin lamina of cerebellar white matter at the rostral roof of the fourth ventricle. From a dorsal brainstem and superior cerebellar viewpoint, the velum appears as a pale triangular sheet that slopes inferomedially toward the ventricular cavity while the peduncles frame it laterally as stout fiber bundles ascending toward the midbrain. Superiorly, the sheet approaches the inferior colliculi and the frenulum of the superior medullary velum, and inferiorly it transitions toward the cerebellar vermis and the cerebellar cortex at the apex of the ventricle. Fine surface texture distinguishes the velum from the more cordlike peduncular margins. Small structure, big consequences. Rostral fourth ventricular anatomy matters whenever you teach CSF pathways, dorsal midbrain landmarks, or the surgical corridors that traverse the cerebellomesencephalic fissure. Lesions in the superior cerebellar peduncle decussation and adjacent dorsal tegmentum, including demyelinating plaques or midbrain tumors extending toward the roof of the fourth ventricle, can be oriented more reliably when the superior medullary velum is treated as a fixed boundary between the peduncles and the ventricular space. The lighting and isolation of white matter planes in this illustration make it easier to separate the velum from the overlying tectum and the surrounding cerebellar folia, a distinction that is often blurred in cadaveric photographs. Neuroanatomy faculty can drop this illustration into a cerebellum and brainstem block to anchor the concept of the fourth ventricle roof, while neurosurgery and neuroradiology teams can use it in case conferences discussing dorsal approaches to the upper fourth ventricle and tectal region.
