Description
Vestibulocerebellum anatomy centers on the flocculonodular lobe, with the paired flocculi positioned anteroinferiorly on the cerebellar hemispheres and the nodulus forming the inferior vermian component at the midline. From an anteroinferior perspective, the flocculus sits lateral to the nodulus near the cerebellopontine angle, while the nodulus lies close to the roof of the fourth ventricle and the foraminal region of Magendie. Adjacent inferior cerebellar surfaces and the contour of the hindbrain provide orientation, placing these structures inferior to the tentorial surface and posterior to the brainstem. Clinically, the flocculonodular lobe is the cerebellar territory most directly tied to vestibular input, so lesions here classically produce truncal ataxia, vertigo, gaze-evoked nystagmus, and impaired vestibulo-ocular reflex adaptation rather than prominent limb dysmetria. Small infarcts in the posterior inferior cerebellar artery distribution, demyelinating plaques, or mass effect in the region of the fourth ventricle can all involve the nodulus and flocculus, and the resulting pattern helps separate vestibulocerebellar syndromes from spinocerebellar or cerebrocerebellar deficits. Orientation matters. The fixed inferior exposure in this illustration makes the spatial relationship between the midline nodulus and the lateral flocculus easy to teach and to reference when correlating symptoms with anatomic locus.
