Description
Tibial anterior border (anterior crest) dominates the frontal view as a sharp, subcutaneous ridge extending inferiorly from the region of the tibial tuberosity along the anterior shaft toward the distal metaphysis and medial malleolus. Medial to the crest, the broad medial surface of the tibial body faces anteromedially, while lateral to it the anterolateral surface slopes toward the interosseous border where the interosseous membrane attaches to the fibula. Proximally, the anterior margin sits inferior to the tibial condyles and adjacent to the tibial tuberosity, a key landmark for the patellar ligament insertion. Easy to palpate. Clinically, the anterior tibial crest is a frequent site of direct-impact pain and periosteal irritation because it lacks a muscular cushion, and it helps clinicians localize fractures of the tibial diaphysis on exam before radiography. Orthopedic discussions of tibial shaft fractures often reference the crest for describing displacement and for orienting fixation, since rotation of the distal fragment is judged by the relationship of the crest to the medial malleolus in anatomic position. This fixed frontal perspective also supports teaching surface anatomy and landmark-guided approaches by isolating the ridge and its continuity along the shin.
