Description
Arising from the superior mesenteric artery, the anterior inferior pancreaticoduodenal artery courses superiorly and to the right, hugging the inferior margin of the pancreatic head before turning to meet the anterior superior pancreaticoduodenal branch from the gastroduodenal artery. An anterior approach places it in front of the uncinate process and adjacent to the second and third parts of the duodenum, with the portal venous confluence and pancreatic neck lying more posterior and medial. The common bile duct and pancreatic duct sit superior within the head region, while the jejunal mesentery and its arcades fan inferiorly from the SMA. Orientation is anchored by the rib cage superiorly, liver in the right upper quadrant, stomach and spleen to the left, and kidneys with adrenal glands retroperitoneally. This arterial arcade matters because it forms one of the main anastomotic channels between the celiac trunk and SMA territories, and it often becomes enlarged in celiac artery stenosis or median arcuate ligament syndrome. Its intimate relationship to the pancreatic head and duodenum makes it a recurrent source of hemorrhage in pancreatitis-related pseudoaneurysm and a vessel at risk during pancreaticoduodenectomy, duodenal surgery, and uncinate process dissection along the SMA plane. Small vessel, big consequences. Use this artwork when teaching foregut to midgut collateral pathways in gross anatomy or surgical anatomy, and when illustrating operative planning for a Whipple procedure, SMA-first approaches, or endovascular management of pancreaticoduodenal aneurysms.
