Anterior thalamic nuclei occupy the rostral pole of the thalamus on this medial view, sitting just inferior to the stria medullaris thalami and immediately anterior to the mediodorsal (dorsomedial) nucleus. Medially, their border approaches the third ventricle, while laterally they are separated from neighboring nuclear groups by the internal medullary lamina as it divides into its characteristic anterior bifurcation. Superiorly the nuclei relate to the thalamic surface beneath the body of the fornix and choroid plexus, and inferiorly they grade toward the hypothalamic region near the massa intermedia when present. Boundaries matter here. Clinical relevance centers on limbic circuitry. The anterior nuclear group forms a key relay in the Papez circuit, receiving mammillothalamic tract input from the mammillary bodies and projecting to the cingulate gyrus, so focal lesions can present with disproportionate anterograde memory impairment and disorientation rather than primary motor or sensory loss. Vascularly, the territory is classically supplied by the tuberothalamic (polar) artery arising from the posterior communicating artery, and a small paramedian or polar thalamic infarct can be anatomically localized on the basis of this anterior and medial topography; the fixed medial perspective makes the relationship to the internal medullary lamina and third ventricular wall easy to teach when discussing clinicoradiologic correlation on axial and coronal MRI. Neuroanatomy courses and board review atlases will benefit from this illustration when explaining thalamic nuclear organization, the Y-shaped internal medullary lamina, and limbic pathways that are often abstracted in schematic diagrams. It also fits well in neurology and neuroradiology teaching files covering thalamic stroke syndromes, diencephalic anatomy, and memory network lesions.
