Posterior anatomy of the male head and neck is rendered with the occipital region and skull base superiorly and the cervical vertebral column centrally. Paired vertebral arteries ascend on either side of the cervical vertebrae, tracking superiorly toward the foramen magnum after coursing through the transverse foramina, while branches of the external carotid system supply the posterior scalp and upper neck. A dense network of smaller arterioles and communicating vessels spans the suboccipital and upper cervical region, draped lateral to the spinous processes and deep to the posterior cervical soft tissues. Bony landmarks include the posterior arch of the atlas, axis, and the remaining cervical vertebrae stacked inferiorly. Posterior vascular orientation matters when you are teaching the vertebrobasilar circulation, because the vertebral artery’s tortuous segment around C1 and its entry into the cranial cavity at the foramen magnum are common points of surgical caution. It also frames the suboccipital region where cervical manipulation, posterior instrumentation, or trauma can endanger the vertebral artery, leading to vertebral artery dissection and posterior circulation stroke.
