Description
Centered on the midline of the posterior cranial fossa, the basilar venous plexus forms a flattened, irregular venous network applied to the clivus, immediately superior to the foramen magnum and anterior to the pontomedullary junction. Inferiorly oriented, the plexus sits between the dura mater and the basiocciput, with venous channels extending laterally toward the inferior petrosal sinuses along the petroclival fissure and posteriorly toward the margins of the foramen magnum. Expect nearby landmarks to include the ventral surface of the pons and medulla, the upper cervical spinal cord, and the attachment region for the tectorial membrane over the clivus. Small but busy anatomy. Venous anatomy at the craniovertebral junction matters because the basilar plexus is a communication hub between the cavernous sinus region, the inferior petrosal sinuses, the marginal sinus, and the internal vertebral venous plexus, creating low-pressure collateral pathways that can redistribute flow when jugular outflow is compromised. This is also a corridor encountered indirectly during endoscopic endonasal and transclival approaches to ventral brainstem lesions, where venous bleeding from the basilar plexus can obscure the operative field despite its deceptively thin, flattened appearance. In infection and tumor spread, this valveless venous network helps explain routes of cranial to spinal venous communication.
